Public Health Nurse II Position

The State of Maine, Bureau of Human Resources is seeking a skilled Public Health Nurse II to join our team. As a Public Health Nurse II, you will play a vital role in promoting and protecting the health of populations using knowledge from nursing, social, and public health sciences.
Key Responsibilities
Apply the nursing process to individuals and families, including health assessment, nursing diagnoses, care plan development, implementation, evaluation, resource coordination, and/or referral(s) to meet demonstrated health care needs.
Observe the home environment, identify health problems or at-risk behavior, and contact appropriate resources and agencies to respond to family and client needs.
Document assessments and interventions in medical records, using established medical record software, forms, and documentation practices.
Plan, develop, and coordinate client care with hospitals, medical providers, schools, and social services agencies to ensure necessary services are accessible.
Participate in response teams to prevent, minimize, and contain adverse health events and conditions resulting from communicable diseases, food-, water-, and vector-borne outbreaks, chronic diseases, environmental hazards, injuries, and health disparities.
Requirements
Bachelor's degree in nursing or graduation from an accredited school of nursing and 2 years of professional nursing experience.
Licensed as a Registered Professional Nurse as issued by the State Board of Nursing.
