This position is a full-time, project position, housed within the Public Health Division of the City of Portland. The ideal candidate will be passionate about healthcare for vulnerable populations of all ages, flexible, and resourceful. Must have good decision making skills and the ability to work independently as well as a part of a team.
Key Responsibilities:
* Serve patients across the clinical services offered at 39 Forest Ave, including New Mainer Screening & Referral, Harm Reduction Services, Immunization Program, STD Clinic, and the Portland Community Free Clinic.
* Coordinate referrals, provide case management, perform telephone intakes and triage, and partner with community healthcare and social service resources during hours when patients are not being seen.
* May also be engaged in education for staff and community partners on best public health practice, reporting, and outreach.
Requirements:
* BSN and one year of experience are preferred but new graduates are encouraged to apply.
* Successful candidates will exhibit excellent organizational, clinical, communication, and interpersonal skills.

* This is a union position, CEBA (Grade 16M) - Starting salary range: $32.32/hr.
