Job Title: LPN - Full Time Evening Shift

Join our team at Ann's Choice by Erickson Senior Living as a Licensed Practical Nurse (LPN) for a full-time evening shift position. As an LPN, you will provide clinical care to residents in our neighborhood, working closely with our interdisciplinary team to ensure the highest quality of care.

Responsibilities:
  • Develop and maintain relationships with residents and their families, providing a consistent caregiver approach that supports their physical, spiritual, emotional, and clinical needs.
  • Assess, observe, and evaluate residents' clinical conditions, documenting findings in their electronic medical record and notifying providers and RNs as necessary.
  • Update or modify care plans as needed to reflect changes in residents' care and service needs.
  • Supervise and lead care associates, ensuring they provide high-quality care to our residents.
  • Administer medications timely, accurately, and according to orders.
  • Participate in meetings related to resident outcomes and care, contributing to the development of effective care plans.
  • Respond to residents, family members, or responsible parties in a timely and professional manner.
  • Participate in the review of clinical indicator reviews, including falls, wounds, nutrition, and antipsychotic use.

Compensation:

$29-$37/hr, plus $2 shift differential. Hourly rate depends on years of experience.

Schedule:

Full Time, 3:00P-11:00PM

W1: SUN, MON, WEDS, THURS, FRI

W2: MON, TUE, WEDS, THURS, SAT

Requirements:
  • Graduate of an approved Licensed Practical/Vocational Nursing program.
  • A minimum of 1 year of experience in assisted living, home health, or long-term care is preferred.
  • A passion to care for seniors and be a strong resident advocate.
  • Experience with electronic medical records is preferred.
