Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Ann's Choice by Erickson Senior Living. As a LPN, you will play a vital role in providing high-quality clinical care to our residents, working closely with our interdisciplinary team to ensure the best possible outcomes.

Key Responsibilities
  • Develop and maintain relationships with residents and their families, providing personalized care and support.
  • Assess and evaluate resident clinical conditions, documenting findings and notifying providers as needed.
  • Collaborate with the care team to update and modify care plans, ensuring resident needs are met.
  • Administer medications accurately and timely, following established protocols.
  • Participate in meetings and reviews to discuss resident outcomes and care.
  • Respond to resident, family, or responsible party inquiries in a timely and professional manner.

Requirements
  • Graduate of an approved LPN program.
  • Minimum 1 year of experience in assisted living, home health, or long-term care.
  • Passion for caring for seniors and advocating for their needs.
  • Experience with electronic medical records preferred.
