LPN/LVN Job Summary

We are seeking a compassionate and skilled LPN/LVN to join our team at Ann's Choice by Erickson Senior Living. As a Licensed Practical Nurse, you will play a vital role in providing high-quality clinical care to our residents in the assisted living neighborhood.

Key Responsibilities:
  • Provide clinical care to residents in accordance with nursing standards of practice, board of nursing, and Erickson Senior Living standards.
  • Develop relationships with residents and their families, using a consistent caregiver approach to support their physical, spiritual, emotional, and clinical needs.
  • Assess, observe, and evaluate residents' clinical condition, documenting findings in the resident electronic medical record and notifying the provider and/or RN as necessary.
  • Update or modify care plans as needed to reflect any changes to a resident's care and service needs.
  • Supervise and lead care associates.
  • Participate in meetings related to resident outcomes and care.
  • Respond to residents, family, or responsible parties in a timely and professional manner.
  • Participate in the review of clinical indicator reviews – falls, wounds, nutrition, antipsychotic use, etc.

Requirements:
  • Graduate of an approved Licensed Practical/Vocational Nursing program.
  • A minimum of 1 year of experience in assisted living, home health, or long-term care is preferred.
  • A passion to care for seniors and be a strong resident advocate.
  • Experience with electronic medical records is preferred.
