LPN/LVN Job Description

At Ann's Choice by Erickson Senior Living, we're committed to providing exceptional care to our residents. As a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN), you'll play a vital role in delivering high-quality clinical care in our assisted living community.

Responsibilities:
  • Provide clinical care to residents in accordance with nursing standards of practice, board of nursing, and Erickson Senior Living standards.
  • Develop relationships with residents and their families, using a consistent caregiver approach to support their physical, spiritual, emotional, and clinical needs.
  • Assess, observe, and evaluate residents' clinical conditions, documenting findings in the resident electronic medical record and notifying providers and/or RNs as necessary.
  • Update or modify care plans as needed to reflect changes in residents' care and service needs.
  • Supervise and lead care associates, administering medications timely, accurately, and according to orders.
  • Participate in meetings related to resident outcomes and care, responding to residents, family, or responsible parties in a timely and professional manner.
  • Participate in the review of clinical indicator reviews, including falls, wounds, nutrition, and antipsychotic use.

Requirements:
  • Graduate of an approved Licensed Practical/Vocational Nursing program.
  • A minimum of 1 year of experience in assisted living, home health, or long-term care is preferred.
  • A passion to care for seniors and be a strong resident advocate.
  • Experience with electronic medical records is preferred.
