Key Responsibilities- Assess patient needs and develop individualized care plans to ensure optimal outcomes.
- Collaborate with healthcare teams to coordinate patient care services and facilitate multidisciplinary decision-making.
- Utilize evidence-based practices to drive quality improvement initiatives and promote best practices in patient care.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions and patient satisfaction.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care.
- Current state RN license and bachelor's degree from an accredited nursing school.
- Basic Life Support certification within 30 days of hire.
- ACLS and PALS certification within six months of hire.
- One year of experience in case management or a related field preferred.
