Clinical Case Manager

About the Role

You'll be accountable for the execution, adaptation, and outcome of care management as prescribed in the initial Utilization Review Plan with regard to resources, appropriateness of care, and adherence to a designated Geometric Length of Stay (GLOS) for each patient. You'll work with physicians, nurses, ancillary staff, and social services at the point of care to facilitate multidisciplinary decision-making that is consistent with the goals and objectives of the plan of care and the wishes of both the patient and family.

Requirements

Applicants should have a current state RN license. A Bachelor's degree is preferred. Additional requirements include Basic Life Support certification within 30 days of hire, ACLS and PALS within six months of hire, and one year of experience preferred. ASLS certification is also preferred. Minimum Work Experience 1-3 years case management experience is preferred. Requires critical thinking skills, decisive judgment, and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
