  • Intermediate to Advanced computer skills, with experience in electronic medical records and Microsoft Office
Preferred Qualifications
  • BSN degree
  • 3-5 years of experience in home assessment and care coordination
  • Experience with health promotion, coaching, and wellness
  • Previous managed care experience
  • Bilingual in English and Spanish
  • Certification in Case Management
Work Environment

This is a field position, requiring travel to members' residences and community locations. The ideal candidate will be able to work independently, with minimal supervision, and possess excellent time management skills.
