Position Overview

The Clinical Pharmacy Manager for Oncology Investigational Drug Service is accountable for the delivery of clinical pharmacy services, advocating for evidence-based and rational medication therapy, and ensuring high-quality pharmaceutical care through the creation and enhancement of pharmacy practice initiatives, medication therapy guidelines, and other relevant programs. This role serves as a vital clinical pharmacy resource within the health system and its associated pharmacy services. The manager will also mentor pharmacy residents and students from nearby pharmacy colleges, engage in research, and strive for publication of findings. Responsibilities include supervising pharmacists to optimize patient care, utilizing independent judgment for staff direction and discipline, and participating in patient care rounds, clinical interventions, medication evaluations, and institutional committees.

Key Responsibilities

1. Execute clinical, technical, service, and administrative duties:

- Engage in departmental initiatives to align with the goals and policies of the department and hospital, adhering to standards set by regulatory bodies.

- Formulate and enforce policies and procedures to uphold professional standards and enhance medication safety, in line with guidelines from the American Society of Health-System Pharmacists (ASHP).

- Support the Pharmacy & Therapeutics committee by preparing drug assessments and aiding in formulary management.

- Collaborate with healthcare professionals to evaluate and optimize medication therapy, ensuring cost-effectiveness and safety.

- Participate in interdisciplinary rounds, providing follow-up on clinical recommendations with prescribers and care teams, ensuring thorough documentation.

- Conduct patient-centered medication therapy management, including reconciliation and counseling, to improve clinical outcomes and minimize readmissions and adverse reactions.

- Maintain confidentiality in patient and employee information management.

- Coordinate pharmaceutical care delivery among pharmacy department staff.

- Ensure a well-maintained work environment and demonstrate leadership.

2. Stay updated with pharmaceutical databases and national guidelines, applying evidence-based practices in medication management.

3. Educate patients and caregivers about medications and disease management, covering:

- Goals of chronic disease management

- Medication details including name, strength, and purpose

- Safe and timely medication administration

- Mechanisms of action and interactions

- Importance of therapeutic drug monitoring

- Recognition of side effects and adherence to therapy

4. Demonstrate the necessary knowledge and skills to provide age-appropriate care across all patient demographics.

5. Foster educational and professional development among pharmacy staff, identifying learning needs and providing training on medication appropriateness, safety, and cost-effectiveness.

6. Oversee pharmacy staff, including interviewing, performance evaluations, and implementing corrective actions as necessary.

Qualifications

- Doctor of Pharmacy Degree required.

- ASHP-Accredited PGY-1 residency required; PGY-2 preferred.

- Board Certification (e.g., BCPS, BCCCP) is strongly encouraged.

- Minimum of 3 years of hospital experience or equivalent clinical training.

- Registered Pharmacist with the NYS Board of Pharmacy, maintaining compliance with regulatory and educational requirements.
