Hospice RN Case Manager

We are seeking a compassionate and skilled RN Case Manager to join our team at Idaho Home Health & Hospice. As a Hospice RN Case Manager, you will play a vital role in delivering high-quality end-of-life care to our patients and their families.
Key Responsibilities:- Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
- Identify patients' and families' physical, psychosocial, and environmental needs, and reassess as needed, no less than every 15 days.
- Initiate and coordinate the plan of care, maintaining accurate and relevant clinical notes regarding patients' conditions.
- Document problems, appropriate goals, interventions, and patients'/families' responses to hospice care.
- Collaborate with patients, families, attending physicians, and other members of the IDG to provide daily patient and family care.
- One year of clinical experience.
- Current RN licensure in the state of practice and one year of clinical experience.
- Current CPR Certification.
- Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
