Key Responsibilities:
  • Conduct initial nursing evaluations to determine patient eligibility for hospice services within 48 hours of referral
  • Identify and address patients' physical, psychosocial, and environmental needs through ongoing assessments and care planning
  • Collaborate with patients, families, attending physicians, and other healthcare professionals to deliver comprehensive care
  • Document patient progress, goals, interventions, and outcomes in a timely and accurate manner
Requirements:
  • Current RN licensure in the state of practice
  • One year of clinical experience
  • CPR certification
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
