Key Responsibilities
  • Clinical Competence
    • Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
    • Supervise and direct the care provided by home health aides and licensed practical vocational nurses, ensuring adherence to state and federal regulations.
  • Documentation and Care Delivery
    • Provide high-quality clinical services within the scope of practice, adhering to infection control standards and payer requirements.
    • Accurately and timely complete comprehensive assessments, including medication reconciliation.
    • Document patient visits according to policy and payer requirements.
  • Quality
    • Conduct initial and comprehensive nursing evaluations, ensuring patients meet home health eligibility and medical necessity guidelines.
    • Develop and implement plans of care within state-specific guidelines, in collaboration with physicians.
    • Communicate relevant information effectively with agency staff, physicians, patients, and family members.
  • Professionalism
    • Arrive at work with a positive attitude, willing to help patients and the agency in any way possible.
    • Participate in all meetings, including case conferences, and actively engage in survey readiness activities.
  • Teamwork
    • Take direction from Clinical Director and Executive Director professionally, completing tasks assigned in a timely manner.
    • Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
    • Participate in on-call rotation and weekend rotation as needed to meet patient needs.
Requirements
  • Current RN licensure in the state of practice.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
