Job Summary

We are seeking a skilled Registered Nurse to join our team at Idaho Home Health & Hospice. As a Registered Nurse, you will provide high-quality patient care in a home health setting, working closely with our interdisciplinary team to deliver exceptional patient outcomes.

Key Responsibilities
  • Clinical Competence
    • Develop and implement individualized care plans in collaboration with the patient's physician and other healthcare professionals.
    • Direct and supervise the care provided by home health aides and licensed practical vocational nurses, ensuring adherence to state and federal regulations.
  • Documentation and Care Delivery
    • Provide comprehensive assessments, including medication reconciliation, and accurately document patient visits according to policy and payer requirements.
    • Deliver high-quality clinical services within the scope of practice, adhering to infection control standards and coordinating with other members of the healthcare team.
  • Quality
    • Ensure patients meet home health eligibility and medical necessity guidelines, accurately determining primary focus of care and developing the plan of care within state-specific guidelines.
    • Communicate relevant information effectively with agency staff, physicians, patients, and family members to ensure quality care and service excellence.
  • Professionalism and Teamwork
    • Exhibit a positive attitude, willingness to learn, and adaptability in a fast-paced environment.
    • Participate in on-call rotation, weekend rotation, and other duties as assigned, demonstrating a commitment to teamwork and patient care.
Requirements
  • Current RN licensure in the state of practice.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
