Job Summary

We are seeking a highly skilled and compassionate Resident Relations Coordinator to join our team at Barton Health. As a key member of our Skilled Nursing department, you will play a vital role in providing exceptional care and support to our residents.

Key Responsibilities
  • Develop and implement social services that meet the physical, mental, and psychosocial needs of our residents.
  • Assess and reassess residents to determine their social service needs and develop a plan to address them.
  • Collaborate with the multidisciplinary team to ensure that care plans accurately reflect resident needs and attainment of goals.
  • Coordinate physician consult visits, dentist, dental hygienist, optometrist, podiatrist, and telehealth consults as needed.
  • Effectively communicate with residents, family members, and facility staff to ensure seamless care transitions.
  • Assist residents with finances, including accuracy of documentation, yearly MediCal and social security benefits, and resident mail.
  • Support the activities and recreational program in a safe and sensitive manner, promoting a comfortable and relaxed atmosphere.
  • File paperwork with Social Security and Medi-Cal, including MC-171 forms, and coordinate with the Activity Director for assessments and reassessments.
Requirements
  • High school diploma or GED required; Bachelor's or Master's degree in Social Services or related field preferred.
  • Previous experience as a Social Worker preferred.
  • Previous computer experience and database management preferred.
  • Sufficient computer skills to complete online applications and Net Learning requirements.
  • In compliance with patient safety standards, must be able to effectively communicate in English; bilingual abilities preferred.
  • Social Services Designee Certification required within 6 months of hire.
  • Current CPR/BLS certification by the American Heart Association.
  • Valid California or Nevada Driver's license or ability to obtain prior to start date.
Working Conditions

The work environment is a normal office setting with occasional travel to various health system locations.

The noise level is usually quiet to moderate while in the office.
