Blue Shield of California is seeking a highly skilled Care Manager to join our team. As a key member of our Care Management team, you will play a critical role in ensuring that our members receive high-quality, cost-effective care.
Key Responsibilities- Develop and implement individualized care plans to promote quality of care and cost-effective health services.
- Conduct thorough assessments of members' health needs and develop targeted interventions to address identified gaps in care.
- Collaborate with healthcare providers, community resources, and other stakeholders to ensure seamless transitions of care.
- Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.
- Provide education and support to members and their families to promote self-management and empowerment.
- Current CA RN License
- Bachelor of Science in Nursing or advanced degree preferred
- Certified Case Manager (CCM) Certification or in process of completing certification
- At least 5 years of prior experience in nursing, healthcare, or related field
- Managed care experience in inpatient, outpatient, or managed care environment preferred
- Excellent communication and interpersonal skills
