Key Responsibilities:

  • Conduct comprehensive assessments of patients' needs to develop individualized care plans

  • Maintain accurate documentation of patient assessments, care plans, and outcomes

Requirements:

  • Graduate of an accredited nursing program and active RN license in California
  • Minimum of 1 year of nursing experience in hospice
