Job Summary

We are seeking a highly skilled Palliative Care Advanced Practice Provider to join our team at Genesis HealthCare, Inc. This is a full-time position that offers a unique opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities
  • Provide primary and urgent care services to nursing center residents in collaboration with the primary care team.
  • Collaborate with the Center Leadership Team to achieve center-specific goals and improve the quality of care.
  • Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients with serious illnesses.
  • Provide education and guidance to patients, families, and healthcare teams on palliative care principles and practices.
  • Participate in interdisciplinary teams to coordinate care and ensure seamless transitions between settings.
Requirements
  • Master's degree in Nursing with a concentration in Older Adult and/or Family Nurse Practitioner.
  • Minimum of two years of experience working with the adult or geriatric population.
  • Current APRN license and DEA certification in each state where practicing.
  • Palliative care certification preferred.
