We are seeking a highly skilled Palliative Care Advanced Practice Provider to join our team at Genesis HealthCare, Inc. This is a full-time position that offers a unique opportunity to make a meaningful difference in the lives of our patients and their families.
Key Responsibilities- Provide primary and urgent care services to nursing center residents in collaboration with the primary care team.
- Work closely with the Center Leadership Team to achieve center-specific goals and objectives.
- Collaborate with the primary care team to provide patient-specific medical care and develop individualized care plans.
- Provide palliative care services to nursing center residents with serious illness, focusing on pain and symptom management, psychosocial support, and advance care planning.
- Serve as a specialized consultant within the interprofessional team, providing recommendations for the plan of care while incorporating patient goals and preferences.
- Graduate from an accredited School of Nursing program and licensed as a registered nurse.
- Certified as a Nurse Practitioner by being a nursing graduate from an accredited Nurse Practitioner Program with a Master's Degree in Nursing and a concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP).
- A minimum of two years experience working with the adult or geriatric population is preferred.
- In states where practice allows; APRN with an unrestricted professional license and DEA, for each state in which they practice.
- Palliative care certification preferred.
- A comprehensive 90-day training program and continued support.
- New and recent graduate mentorship and clinical education program.
