Nursing Care Management Specialist
Annual Compensation:
Level I: - Y15A - $58, Comprehensive State Employee Benefits
Level II: - Y15B - $62, Comprehensive State Employee Benefits
Occasional travel required - A valid driver's license and maintenance of necessary vehicle insurance are mandatory.
Essential Qualifications:
Level I:
- Valid permanent Oklahoma nursing license as sanctioned by the Oklahoma Board of Nursing or Registered Nurse Licensure Compact (eNLC)
- Minimum of two years in professional nursing practice
Level II:
- Valid permanent Oklahoma nursing license as sanctioned by the Oklahoma Board of Nursing or Registered Nurse Licensure Compact (eNLC)
- Minimum of three years in professional nursing practice
Key Responsibilities
- Evaluates the health needs of assigned service recipients and collaborates with case managers to identify health risks, providing recommendations prior to the annual Individual Planning (IP) meeting through the following tasks:
a. Conducts a Health Review and updates the Medical Event, Diagnosis, Allergy, Medication, and Provider sections in the Care Coordination Management (CCM) system.
b. Analyzes documentation such as home records, CCM, DDS-5, DDS-6, Incident Reports, IP, and electronic communications to assist case managers in recognizing health risks/concerns of service recipients.
c. Prepares and submits a written Health Review to the case manager with recommendations for developing the Health Section of the IP.
d. Formulates a Nursing Service Support Plan as necessary.
e. Submits Emergency PASRR assessments to the state office within 5 calendar days.
f. Submits Routine and Reconsideration PASRR assessments to the state office within 7 working days.
g. Completes the Initial 24 Hour Mortality Report within 24 hours of notification of a service recipient's death and submits it to the Nurse Manager and state office.
h. The Mortality Reviewer finalizes the Mortality Report and facilitates the Mortality Review Committee meeting within 30 days of receiving records.
i. Manages transitions and transfers of service recipients.
