BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team on a per diem basis in East York, Pennsylvania.
Responsibilities- Conduct home visits to clients in designated geographic territories to provide rehabilitative care and skilled nursing services.
- Perform diagnostic tests and measurements to assess client mobility, strength, and balance.
- Develop and implement individualized care plans to promote client health, safety, and independent living.
- Educate and instruct clients, family members, or caregivers on rehabilitative care and activities necessary for optimal recovery.
- Accurately document client care management and services provided using a state-of-the-art touch pad tablet.
- A current PA Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
