Participant Engagement and Advocacy

  2. Teach self-advocacy skills and act as a liaison with healthcare providers and family members.
  3. Assist participants with daily living activities, promoting healthy lifestyle habits.
  4. Support educational pursuits by helping participants explore interests and access relevant resources.
  5. Facilitate money management training and track in-house finances accurately.
  6. Encourage participant involvement in household tasks to maintain a clean and safe environment.
  7. Provide transportation for participants to essential appointments and activities.
  8. Utilize online reporting systems and electronic health records as necessary.
  9. Lead psycho-educational groups and house meetings.

Health and Wellbeing Support

  1. Utilize a strengths-based approach to assist participants in identifying their goals and collaborating with the support team.
  2. Recognize and manage triggers to prevent crisis situations.
  3. Respond to crises and medical events according to established safety plans.
  4. Adjust support levels based on individual participant needs.
  5. Report any incidents of participant abuse or neglect to appropriate authorities.
  6. Administer medications in compliance with established standards.

Team Collaboration and Continuous Improvement

  1. Actively participate in team meetings and training sessions to enhance job knowledge.
  2. Foster a collaborative team environment focused on participant needs.
  3. Utilize feedback from supervisors to improve professional skills.

Qualifications:

  • Valid driver's license and reliable transportation during work hours.
  • Ability to work independently and collaboratively within a team.

  • Compassionate and patient demeanor.
  • Certification in medication administration (MAP), CPR, and first aid is required.

Education and Experience:

  • High school diploma or equivalent; a degree in human services is preferred.
  • Minimum of six months of relevant experience required.
