Job Overview

TEAM-LLC is in search of a dedicated Nurse Practitioner with expertise in Adult Hab Waiver for a full-time role. Our thriving program collaborates closely with the State of Nebraska Medicaid Waiver (DD) to deliver exceptional services to individuals aged 18 and above with developmental disabilities. Key Responsibilities

  • Develop and implement training materials and competency assessments for medication aide classes tailored to Direct care staff in accordance with Nebraska regulations.
  • Maintain and update medication lists on the Therap platform.
  • Effectively communicate health and nursing needs to staff, healthcare providers, and families.
  • Coordinate hospital admissions, discharges, and necessary follow-ups.
  • Regularly inspect program/service areas to ensure adequate medical and nursing supplies are available.
  • Assist with medical and health appointments as required.
  • Provide direct care services within the treatment or habilitation environment as needed.
  • Ensure nursing and health services comply with state and federal regulations and divisional directives.
  • Administer nursing services as prescribed by physicians and other healthcare professionals.
  • Monitor individual health status through comprehensive nursing services.
  • Document incidents involving individuals and ensure timely and accurate incident reports are completed.
  • Manage individual health needs proactively in response to adverse changes in medical status.
  • Collaborate with home/facility staff regarding psychotropic medications and medication review planning.
  • Track and monitor the height and weight of individuals served.
  • Administer and oversee medications and physical treatments as ordered, assessing for any adverse effects.
  • Report significant changes in individual health status to the attending physician and relevant team members promptly.
  • Screen individuals who are ill and refer them to a physician when necessary.
  • Order, review, monitor, and discontinue medications as directed by physicians, ensuring proper disposal of unused medications.
  • Ensure that monthly Medication Administration Records (MARs) are accurately maintained in each individual's record.
  • Report potential communicable diseases and collaborate with nursing staff and healthcare professionals to address health concerns.
  • Document individual health status routinely in nursing notes.
  • Transcribe physician orders and other medical reports accurately for filing in individual charts.
  • Be available for on-call duties as required.

Qualifications

  • Current and valid nursing license in good standing with the State of Nebraska.
  • Proven ability to develop training materials and competency assessments for medication aide classes per Nebraska regulations.
  • Experience with DD Waiver programs.
  • Strong interpersonal and written communication skills.
  • Commitment to delivering high-quality care.
  • Familiarity with the Therap platform.
