Job Summary

The Navigator plays a vital role in ensuring seamless access to essential appointments and services for clients. Working closely with the case manager, the Navigator provides transportation and logistical support, guaranteeing timely and safe travel to appointments and/or other services. This position addresses the Social Determinants of Health (SDOH) needs of the client, while fostering positive relationships and promoting a supportive environment.

The role is responsible for:

  • Reviewing appointments for the following shift, communicating with medical, case management, and the client.
  • Gathering, printing, and reviewing all necessary or related documents for upcoming appointments.
  • Transporting patients to necessary appointments, government agencies, and medical or behavioral health facilities.
  • Maintaining the schedule for recurring group appointments.
  • Maintaining community connections for efficient coordination of care.
  • Communicating and cooperating with police, probation officers, governmental agencies, nonprofit organizations, and other organizations as needed.
  • Serving as a guide for the client and applying knowledge related to navigating the criminal justice system.
  • Maintaining communication with the care manager(s) and care coordination team for reliable transportation and task assignment. An expectation of the role is to check in with the team upon arrival to and prior to departure from the clinic.
  • Facilitating groups as needed.
  • Documenting all client interactions in the Electronic Medical Record (EMR) in an accurate and timely manner.
  • Other related duties as assigned by the leadership team.

Occasional overtime may be required, and the role may sometimes necessitate work past 5 PM and on weekends.

Physical Requirements


• Lifting up to 50 pounds may be required infrequently.


• This job may require being outside in extreme weather for limited periods of time.


• This job may have exposure to bodily fluids: Personal Protective Equipment (PPE) is provided by the company.


• This type of work involves sitting, standing, walking, bending, and stooping from brief periods of time to lengthy periods of time.


• This position may require ascension and descension of one or more flights of stairs throughout the shift.


• This job will require driving at various times of the day with multiple people in the vehicle.


• This job may require personnel to work weekends or nights. This job may have the opportunity for overtime shifts.


• To perform this job successfully, an individual must be able to perform each essential job duty satisfactorily.

Position Qualifications
  • Minimum of high school diploma or GED.
  • Minimum of 6 months of experience in working in the field of substance abuse treatment or behavioral health-related field (+ 90 days of continuous employment with NorthSight Recovery for internal applications).
  • Must be at least 21 years of age.
  • Clean driving record.
  • Driver's License with no restrictions.
  • Complete all required documentation in accordance with company policies and state criteria.
