Job Summary

Cleveland Clinic's Fairview Hospital is seeking a skilled Registered Nurse (RN) Case Manager to join our team. As a RN Case Manager, you will work collaboratively with inter-professional teams to facilitate effective transitional care coordination.

Key Responsibilities
  • Ensure compliance with accrediting bodies, state mandates, and CMS Conditions of Participation
  • Complete effective handoffs to the next level of care or community to ensure continuity of care
  • Proactively assess, identify, and mitigate risks posed by barriers to patients' transitions of care
  • Work with patients and/or care partners to develop comprehensive, individualized transitional care plans
  • Participate and lead in patient rounding activities, huddles, and committee meetings
Requirements
  • Graduate from an accredited school of professional nursing
  • Current state licensure as a Registered Nurse (RN)
  • Basic Life Support (BLS) certification through the American Heart Association (AHA)
  • 3 years of recent RN clinical experience
Preferred Qualifications
  • Bachelor of Science in Nursing (BSN)
  • Certification by an approved entity within one year of eligibility (CTM, ACM, CCM, or AHA)
  • 2 years of recent experience in utilization review, quality, or care management
  • Knowledge of acute care and accreditation requirements
  • Experience in discharge planning and quality improvement
