RN Case Manager Home Health

AccentCare is seeking a compassionate and skilled RN Case Manager to join our team. As a RN Case Manager, you will play a vital role in delivering high-quality patient care and improving health outcomes in the comfort of patients' homes.

This position is paid on a per-visit basis. The compensation reflected on this posting is an estimate of annual compensation.

To be successful as a RN Case Manager, you will need to have:
- Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation
- One (1) year experience as a RN
You will need to possess and maintain valid CPR certification while employed in a clinical role. Additionally, you must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
You will also need to be able to travel to all business locations.
