VNS Health is seeking a skilled Physical Therapist to join our team in Englewood Cliffs, New Jersey.
About the Role:We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our patients in their homes and communities. As a Physical Therapist with VNS Health, you will have the opportunity to work independently as part of an interdisciplinary care team, conducting comprehensive assessments and developing personalized rehabilitation therapy plans for patients recovering from surgery, illness, or accident.
Responsibilities:- Practice independently in the community as part of an interdisciplinary care team.
- Conducts comprehensive assessment of patients by determining patient needs, develops courses of action including treatment plan and outcomes.
- Deliver personalized rehabilitation therapy to patients in their home or care facilities.
- Constantly evaluate evolving patient needs and respond with plan of care adjustments.
- Minimum of one-year physical therapy experience or successful completion of probationary period as a VNS Health Rehab Intern.
- New York State License and current registration in Physical Therapy.
- Valid driver's license will be required.

If you are applying to the per diem per visit version of this job, the hiring range is as follows: $70- $75 per visit.
