We are seeking a skilled Home Health Physical Therapist to join our team at VNA Health Group. As a Physical Therapist, you will play a vital role in providing high-quality care to patients in their homes or alternate community settings.
Responsibilities- Assessment and Evaluation: Conduct thorough assessments and evaluations of patients to identify their physical therapy needs and develop personalized plans of action.
- Treatment and Intervention: Provide evidence-based physical therapy interventions to patients, including exercises, modalities, and education, to promote optimal recovery and function.
- Documentation and Communication: Accurately document patient progress and outcomes, and communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Quality and Safety: Adhere to agency infection control and safety policies, and maintain a safe and healthy work environment.
- Education and Licensure: Physical Therapy Licensure in NJ required.
- Experience: 2-3 years of prior clinical experience and community-based experience preferred.
- Skills: Strong computer skills, strong communication and interpersonal skills, and a valid NJ driver's license.
