VNS Health is seeking a skilled Physical Therapist to join our team as a Home Care Specialist. As a key member of our interdisciplinary care team, you will provide personalized rehabilitation therapy to patients in their homes or care facilities.
Key Responsibilities- Conduct comprehensive assessments of patients to determine their needs and develop individualized treatment plans.
- Deliver high-quality rehabilitation therapy services to patients, including physical therapy, education, and support.
- Collaborate with other healthcare professionals to ensure seamless care coordination and patient outcomes.
- Monitor patient progress and adjust treatment plans as needed to ensure optimal results.
- Meet productivity and quality standards, as well as adhere to VNS Health's policies and procedures.
- Minimum of one year of experience as a Physical Therapist, preferably in a home care or community-based setting.
- New York State License and current registration in Physical Therapy.
- Valid driver's license or NYS Non-Driver photo ID card (may be required).
- A competitive hourly rate of $70-$75 per visit.
