Key Responsibilities- Conduct comprehensive assessments of patients to determine their needs and develop personalized treatment plans.
- Deliver rehabilitation therapy to patients in their homes or care facilities, focusing on their physical and functional abilities.
- Evaluate patient progress and adjust treatment plans as needed to ensure optimal outcomes.
- Collaborate with interdisciplinary care teams to provide comprehensive care and support to patients.
- Minimum of one year of experience as a Physical Therapist or successful completion of a probationary period as a VNS Health Rehab Intern.
- Licensure to practice as a Physical Therapist in New York State.
- Valid driver's license required for all areas outside the borough of Manhattan.
- Competitive salary range of $85,000 to $127,500 annually.
