Job Summary:The Advanced Practice Healthcare Provider will be responsible for providing primary and specialty care to patients in designated areas, including patient diagnoses, treatment, education, and referrals. This role will also serve as a resource and role model for clinical practice, training, evaluation, and direct management of practitioners and students.
Responsibilities:- Obtain patient histories, perform physical exams, make assessments, and order tests to determine diagnoses and plan of care.
- Record health history, findings, treatment, and recommendations accurately and clearly.
- Consult with supervising physicians and refer patients as necessary to ensure optimal patient outcomes.
- Include patients and families in determining the plan of care and educate them to ensure self-management, follow-up, and compliance with established treatment.
- Support interdisciplinary collaboration to ensure continuous quality care and promote positive health outcomes.
- Bachelor's Degree from an accredited college or university and successful completion of an accredited Physician Assistant Program or Master's Degree in Nursing accredited through the Commission on Collegiate Nursing Education (CCNE) or the Accreditation Commission for Education in Nursing, Inc. (ACEN).
- 3+ years of clinical experience as an Advanced Practice Provider (APP) performing the same or similar job duties.
- Current licensure and certification as required by the Medical Staff Bylaws, Medical Staff Rules and Regulations, and Credentialing Policies/Procedures.
- Active certification as a Nurse Practitioner by one of the following: Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), Gerontology Nurse Practitioner (GNP), Acute Care Nurse Practitioner (ACNP), Pediatric Nurse Practitioner Primary Care (PNP-PC), or Neonatal Nurse Practitioner (NNP).
- Effective leadership skills, including motivation, recruitment, retention, and change management.
