Description

Job Summary:

The High Risk Navigator-Social Services plays a critical role in strengthening community partnerships to bridge healthcare and housing services. This position coordinates care and leverages resources to improve outcomes for individuals with complex needs.

Responsibilities:

  • Convene bi-monthly Coordinated Care Team meetings to coordinate care and leverage existing community-based resources.
  • Provide liaison services between local hospitals, outpatient clinics, emergency rooms, outreach teams, and housing providers to better coordinate care for individuals with complex needs.
  • Provide outreach as needed to Community Care Team referrals.
  • Bring together hospital inpatient and emergency room staff, outpatient care providers, and housing providers in periodic case presentation meetings to identify duplication in services, leverage resources, and coordinate care.
  • Define cross-agency roles and solidify partnerships.
  • Serve as a Community Care Team representative in local, regional, and statewide meetings.
  • Establish policies and protocols to expedite access to services as needed and set up mechanisms for effective follow-up.
  • Collect and manage data on Community Care Team patient reviews, care plans, demographics, and outcomes.
  • Identify leaders and directors of care management or frequent use initiatives in hospitals who can broker high-level partnerships bridging local government, social service providers, housing organizations, and hospitals.
  • Work with the local implementation team to ensure program goals are being met.
  • Fulfill all compliance responsibilities related to the position.

Requirements:

  • Bachelor's degree in a related field.
  • Knowledge of healthcare fields and supportive housing.
  • Strong leadership and communication skills, written and verbal.
  • Excellent organizational skills.
  • Ability to work well with multi-disciplinary service professionals.
  • Good computer skills.

Preferred Qualifications:

  • Master's degree in social services, healthcare, public administration, or policy.
  • Bilingual ability (English/Spanish).
