ResponsibilitiesPosition Summary:
Under the guidance of the Supervisor Care Manager, the incumbent will assess and evaluate potential clients holistically, identifying functional limitations that hinder independent living. This role requires a compassionate approach to understanding each client's unique needs.
QualificationsMinimum Qualifications:
A minimum of two years of experience in delivering social services within the fields of aging, health, and long-term care is essential. A Bachelor’s degree in social work from an accredited institution is required (BSW). A valid California Driver's license is necessary. BLS Certification from an American Heart Association program is mandatory. Strong ability to connect with clients and their families is crucial. Exceptional communication and presentation skills are required.
