Position: Nurse Care Coordinator

Employment Type: Full Time Days

Compensation: Experience Based Rate starting at $79,710/year

United Methodist Communities, a distinguished senior living organization, is in search of a dedicated Nurse Care Coordinator to join our team.


Qualifications:


- Current NJ Licensed Practical Nurse (LPN) or a Bachelor's degree in a relevant field from an accredited institution.
- Preference for candidates with a current NJ Registered Nurse License.
- Minimum of one year experience in case management marketing; two years preferred.
- Proficiency in Microsoft Office Suite.


Key Responsibilities:
  • Collect and assess all referral information, evaluating potential residents for admission.
  • Make informed admission decisions and collaborate with nursing staff on healthcare admissions.
  • Develop and sustain a referral network, conducting site visits as necessary.
  • Facilitate the admission process into healthcare, ensuring timely completion of necessary paperwork.
  • Verify Medicare coverage and other applicable benefits.
  • Engage in care conferences and other required meetings.
  • Follow up with residents post-discharge to ensure well-being, including medication reconciliation and psychosocial assessments.
  • Participate in post-acute discharge activities at home.
  • Ensure timely completion of departmental reports.
