Qualifications:
- Must possess a current NJ Licensed Practical Nurse (LPN) or a Bachelor's degree in a relevant field from an accredited institution.
- Preferred: Current NJ Registered Nurse License.
- Minimum of one year of experience in case management; two years preferred.
- Proficient in Microsoft Office Suite.
Key Responsibilities:
- Collect and assess referral information, evaluating potential residents for admission.
- Make informed admission decisions in collaboration with nursing staff.
- Develop and maintain a robust referral network, conducting site visits as necessary.
- Facilitate the admission process, ensuring all documentation is completed promptly.
- Verify Medicare coverage and other applicable benefits.
- Participate in care conferences and relevant meetings.
- Conduct follow-ups with residents post-discharge to ensure their well-being, including medication reconciliation and psychosocial assessments.
- Engage in post-acute discharge activities at home.
- Ensure timely completion of departmental reports.
