Key Responsibilities- Care Planning and Coordination
- Develop and implement individualized care plans that promote person-centered care and meet the unique needs of each resident.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Leadership and Supervision
- Supervise and mentor licensed nursing staff to ensure high-quality care and adherence to policies and procedures.
- Provide guidance and support to staff to promote a positive and inclusive work environment.
- Quality Improvement and Compliance
- Monitor and analyze quality metrics to identify areas for improvement and implement strategies to enhance resident care and satisfaction.
- Ensure compliance with regulatory requirements and organizational policies.
- Communication and Collaboration
- Communicate effectively with residents, families, and healthcare team members to ensure seamless care transitions and resident satisfaction.
- Collaborate with other departments to ensure a cohesive and resident-centered approach to care.
- Graduate of an approved school of nursing.
- Currently licensed as a RN in good standing with the NJ Board of Nursing.
- Experience in person-centered care and geriatric nursing.
- Previous experience with MDS and geriatric residents desirable.
- Experience with Point Click Care.
- Current CPR Certification and I.V. Certification.
