Care Coordinator Job Description

We are seeking a skilled Care Coordinator to join our team at Texas Children's Hospital. As a Care Coordinator, you will play a critical role in ensuring the optimal care and well-being of our patients.

Key Responsibilities:
  • Assess, develop, implement, and monitor comprehensive care plans in collaboration with patients, families, and healthcare teams.
  • Coordinate and manage care transitions to ensure seamless patient care across the continuum.
  • Collaborate with medical and nursing staff to ensure accurate and timely clinical documentation.
  • Identify and mitigate barriers to care, and develop strategies to improve patient outcomes.
  • Participate in quality improvement initiatives to enhance patient care and safety.
Requirements:
  • Bachelor's Degree in Nursing or an Associate's Degree in Nursing with enrollment in a BSN program.
  • 3 years of nursing experience in an acute care setting.
  • BLS certification from the American Heart Association.
Preferred Qualifications:
  • Experience in healthcare, health education, or a related field.
  • PEARS, CCM, or ACM certification.
  • CCTM certification from the Medical-Surgical Nursing Certification Board.
