Job Summary
We are seeking an Inpatient Care Coordinator II to support our team in creating seamless transitions for hospitalized patients. As a key member of our case management team, you will facilitate customer satisfaction, document patient information in electronic medical records, and focus on discharging patients into lower levels of care.

Responsibilities

  • Review daily hospital census and enter new admissions into Essette prior to 9:50 am
  • Notify the inpatient case manager of admission prior to 9:50 am or call hospitals for discharge dates and enter them into Essette prior to 9:50 am
  • Enter all referral requests and approve referrals for coordinators
  • Document pertinent information in IDX or Essette
  • Manage the inpatient call queue (high volume)
  • Manage work voice mail and respond to messages within 24 hours
  • Notify the discharge coordinator of discharges
  • Facilitate referrals for patient education to internal resources

Requirements

  • High school diploma or equivalent required
  • Customer service or medical experience preferred
  • HMO/Medical Group experience preferred
  • Certified Medical Assistant preferred

Compensation
The estimated annual salary for this role is $55,000 - $65,000, depending on location and experience.
