Job Summary

The LPN Patient Care Manager Assistant is a crucial role at LHC Group, responsible for supporting the Patient Care Manager in delivering high-quality care to home health patients. This position involves coordinating with a team of staff to ensure seamless care delivery, monitoring patient status, and facilitating communication with healthcare providers.

About the Role

As an LPN Patient Care Manager Assistant, you will assist with phone calls from field staff, physician offices, referral sources, patients, and take messages and orders as needed. You will also monitor incoming faxes, coordinate communication, and facilitate document exchange with physician offices. Additionally, you will monitor patient status in hospitals, ensure adequate care coordination, and perform follow-up actions in response to health check calls or tele-monitoring alerts.

Requirements

  • Current LPN licensure in state of practice
  • Current CPR certification

Compensation and Benefits

The estimated salary range for this role is $55,000 - $65,000 per year, depending on location and experience. LHC Group offers a comprehensive benefits package, including medical, dental, and vision coverage, 401(k) matching, and paid time off.
