We are seeking a skilled Clinical Care Coordinator RN to join our team. As a Clinical Care Coordinator, you will play a vital role in managing patients' specialized needs and collaborating with physicians to develop effective care plans.

Responsibilities
  • Evaluate patient needs and develop comprehensive care management plans in conjunction with primary care physicians and specialists.
  • Monitor and assess the effectiveness of care plans, making adjustments as necessary to ensure optimal patient outcomes.
  • Coordinate interdisciplinary approaches to providing continuity of care, including utilization management, transfer coordination, discharge planning, and obtaining authorizations and approvals for outside services.
  • Serve as a clinical liaison with outside agencies, such as County CCS, non-plan facilities, employers, and third-party administrators.
  • Prepare reports, communicate program changes to staff, and develop protocols in accordance with state regulations.
  • Act as a patient advocate and educator, empowering patients to take an active role in their care and treatment.
  • Develop individualized education plans focused on self-management and deliver patient/family education specific to disease states.
  • Collaborate with internal and external physicians, healthcare providers, discharge planners, and outside agencies to ensure seamless care transitions.
  • Facilitate transportation and housing arrangements for patients when needed, and coordinate transmission of clinical and benefit information to patients, families, and outside agencies.
  • Participate in data collection and analysis of clinical outcomes and customer satisfaction standards, contributing to the formulation and implementation of action strategies and outcomes of care or customer service.
Requirements

To be successful in this role, you will need:

  • A minimum of two (2) years of clinical experience as an RN in an acute care or ambulatory care setting.
  • A Bachelor's degree or equivalent experience of four (4) years.
