Job Title: Home Health Case Manager

At Infojini, we are seeking a highly skilled and compassionate Home Health Case Manager to join our team. As a key member of our healthcare team, you will play a vital role in delivering high-quality patient care and ensuring the continuity of care for our patients.

Responsibilities:
  • Assessment and Planning: Conduct thorough assessments of patients in stable and emergent situations to identify their physical, psychosocial, and environmental needs. Develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
  • Data Collection and Analysis: Obtain data on physical, psychological, social, spiritual, and cultural factors that may influence patient/family health status and incorporate that data into the Plan of Care (POC).
  • Documentation and Record-Keeping: Maintain accurate and up-to-date patient records, including visits, contacts, problems, plans, actions, and goals.
  • Infection Control and Safety: Provide care utilizing infection control measures that protect both staff and patients (OSHA).
  • Patient Education and Support: Educate patients and/or their families regarding care, diet, medications, home safety, etc. Use teaching methods appropriate to the age, condition, culture, and assessed learning-readiness of the patient and/or family members.
  • Supervision and Clinical Direction: Supervise and provide clinical directions to Home Health Aides/Home Health Aides/ Licensed Vocational Nurses to ensure quality and continuity of service provided.
  • Pain Management: Assess and reassess pain, utilize appropriate pain management techniques, and educate the patient and family regarding pain management.
  • Referrals and Coordination: Arrange referrals to appropriate agencies when needed.
  • Compliance and Monitoring: Monitor assigned cases to ensure compliance with requirements of third-party payers.
Requirements:
  • Homecare Homebase Software Experience: Required
  • OASIS Experience: Required
  • Current State License as a Registered Nurse: Required
  • CHPN Certification: Preferred
  • Proof of Current CPR Certification: Required
  • Current Driver's License: Required
  • Reliable Transportation and Insurance: Required
  • 1-2 Years Experience as a Registered Nurse in a Clinical Care Setting: Required
  • 2+ Years Home Health Experience: Required
