Responsibilities:

  • Provide quality skilled nursing care to residents, including administering medications, wound care, and other treatments as needed.
  • Document accurately and efficiently in our electronic medical record (EMR) system.
  • Collaborate with our Registered Nurses (RNs) and other healthcare professionals to ensure seamless care coordination.
  • Communicate effectively with clients, families, and other stakeholders to ensure their needs are met.
  • Participate in our quality assurance program to ensure the highest standards of care.

Requirements:

  • Active NYS Licensed Practical Nurse licensure/registration.
  • Active NYS Driver's License, CPR/BLS Card.
  • One year of Licensed Practical Nurse experience preferred, but new graduates are welcome to apply.
  • Previous homecare experience and/or acute care or rehab experience is a plus.
