Key Responsibilities
  • Conduct thorough clinical assessments to identify member needs and develop individualized care plans
  • Collaborate with healthcare providers, members, and families to ensure comprehensive care and support
  • Coordinate transitions of care, including hospital discharges, to prevent readmissions and ensure continuity of care
  • Develop and implement plans to address member psychosocial needs, including referrals to community resources and social services
  • Communicate effectively with healthcare teams, members, and families to ensure timely and accurate information exchange
  • Document all interactions and care activities in the member's medical record, adhering to regulatory and company standards
Requirements
  • Current CA RN License
  • Triage experience preferred
  • Acute Care or E.R. experience preferred
  • Strong communication, critical thinking, and multitasking skills
