Outpatient Case Manager RN/LVN

We are seeking a highly skilled and compassionate Outpatient Case Manager RN/LVN to join our team at Regal Medical Group. As a key member of our healthcare team, you will be responsible for assessing, planning, implementing, and evaluating the care of our patients to ensure they receive the highest quality of care.
Key Responsibilities- Conduct thorough assessments of patients to identify their medical, environmental, and psychosocial needs.
- Develop and implement individualized care plans that address the patient's needs and goals.
- Collaborate with healthcare providers, patients, and families to ensure seamless care coordination.
- Monitor and evaluate patient progress, making adjustments to care plans as needed.
- Communicate effectively with patients, families, and healthcare teams to ensure timely and accurate information exchange.
- Identify and address potential health risks and barriers to care, developing strategies to mitigate these issues.
- Participate in interdisciplinary care rounds to ensure comprehensive care planning.
- Document patient information accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.
- Graduate from an accredited Registered Nursing Program or Licensed Vocational Nursing Program.
- Current CA RN or LVN license, current CPR certification, and valid CA Driver's license.
- 3 years acute care or case management experience preferred.
- 2-3 years of utilization or HMO experience preferred.
- Typing 40 words per minute with accuracy.
