Job Summary

As a Life Planning Specialist, you will be responsible for developing and implementing person-centered Life Plans that reflect the unique needs and aspirations of our members. This will involve working closely with interdisciplinary teams to ensure comprehensive care and support. You will also be required to maintain ongoing contact with critical stakeholders and ensure timely submission of documentation.

In addition to your technical skills and knowledge, you must possess excellent communication and interpersonal skills, as well as strong organizational abilities. A Bachelor's degree or higher, combined with 2+ years of relevant experience, is required for this position.

We offer a competitive salary range of $31.05-$33.49 per hour, depending on experience and qualifications. Benefits include health, vision, and dental insurance, paid time off, tuition reimbursement, and professional development opportunities.
