Union Hospital, a leading healthcare provider in the Wabash Valley, is seeking a skilled Nurse Practitioner to join our Population Health team. As a key member of our team, you will play a vital role in providing high-quality patient care and supporting overall patient wellness.
Job SummaryThe Population Health Nurse Practitioner will work closely with our post-acute care RN navigator to optimize patient management and reduce hospital readmissions. This role requires a strong understanding of population health principles and the ability to collaborate with healthcare professionals to advance coordinated care.
Responsibilities- Perform history, physical exams, and preventive health measures under physician collaboration.
- Collaborate with SNF and ACO Medical Directors to enhance coordinated care and advance directive recommendations.
- Prepare formal care plans and length of stay estimates for patients as necessary.
- Assist in completing patient Advanced Directives and educate nursing home staff on patient wishes.
- Coordinate with healthcare professionals, manage at-risk patients, reduce hospital readmissions, and ensure medication reconciliation.
- Graduate from an accredited MSN program with certification from ANCC or AANP or obtain certification upon hire.
- Must have Indiana RN license.
- Must have active APRN/CSR license, or be eligible for licenses.
