Job Summary

Doylestown Health is seeking a highly skilled Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in ensuring seamless transitions of care for our patients, while promoting optimal health outcomes and patient satisfaction.

Key Responsibilities
  • Discharge Planning: Develop and implement individualized discharge plans in collaboration with the multidisciplinary team, patients, and families.
  • Care Coordination: Facilitate communication among healthcare providers, patients, and families to ensure timely and effective care transitions.
  • Advocacy: Act as a patient advocate, promoting their rights and interests while ensuring informed decision-making.
  • Data Collection: Participate in quality improvement activities and risk management initiatives, contributing to the development of best practices.
  • Professional Conduct: Demonstrate exceptional professionalism, respect, and empathy in all interactions with patients, families, and healthcare staff.
Requirements
  • Education: Bachelor's degree in Nursing, Social Work, or related field.
  • Experience: Minimum of three years in acute care, with experience in case management, utilization review, or discharge planning.
  • Certifications: Certification in Case Management (CCM) or equivalent preferred.
