Patient Care Coordinator

Position Overview:
The Patient Care Coordinator plays a vital role in ensuring effective collaboration among various stakeholders, including patients, families, and the interdisciplinary healthcare team. This position is dedicated to facilitating and coordinating services tailored to the unique needs of patients throughout their care journey, particularly during transitions from hospital to home or other care settings.
Key Responsibilities:
- Care Transition Planning: The coordinator will assess and reassess patient needs on a daily basis through interviews and review of medical records. Care transition planning begins upon admission, with a preliminary plan documented within 24-48 hours. This plan is developed in partnership with the healthcare team and incorporates input from patients and their families. Identifying potential obstacles is crucial for ensuring a seamless transition to subsequent care levels.
- Facilitation of Care Activities: The coordinator will proactively address issues related to the development and execution of care plans. All activities and discussions must be meticulously documented in the patient's medical records. This may involve communication with external parties, such as primary care providers, to eliminate barriers to care and achieve optimal patient outcomes. The coordinator is responsible for verifying insurance benefits, obtaining necessary authorizations, and identifying community resources as required.
- Patient Advocacy: Advocacy is a core component of the coordinator's role, ensuring that patients or their designated representatives fully understand the care options available. This may involve mediating discussions between healthcare providers and families in situations where conflicts arise.
- Data Collection and Quality Improvement: The Patient Care Coordinator will engage in gathering data related to quality improvement initiatives and risk management efforts.
- Professional Conduct: The coordinator is expected to maintain a high standard of professionalism in all interactions with patients, families, medical staff, payers, and colleagues, while adhering to confidentiality regulations. Accountability for mistakes and prompt corrective actions are essential.
Qualifications:
- Educational Background: Candidates may be selected from one of three professional backgrounds: Registered Nurse, Master’s Level Social Worker, or Bachelor’s Level Social Worker.
- Experience Requirements: A minimum of three years of experience in an acute care environment is required for all candidates, with specific experience in case management or related fields preferred.
- Skills and Competencies: Strong communication and organizational skills are essential, along with a solid understanding of managed care systems and utilization review criteria. Knowledge of community resources, leadership capabilities, and familiarity with information systems are also important. Certification in Case Management (CCM) is preferred for all candidates.
