We are seeking an experienced Registered Nurse to join our team at Heart of Hospice. As a Hospice RN, you will play a vital role in providing end-of-life care to patients and their families.
Job SummaryThe ideal candidate will have a strong background in nursing and excellent communication skills. They will be responsible for assessing patient needs, developing personalized care plans, and collaborating with healthcare teams to ensure high-quality care.
Responsibilities- Evaluate patient needs and develop individualized care plans
- Collaborate with healthcare teams to ensure comprehensive care
- Provide emotional support and education to patients and families
- Maintain accurate and detailed clinical records
- 1 year of clinical experience as a Registered Nurse
- Current RN licensure in the state of practice
- CPR certification
- Valid driver's license and reliable transportation
