About the Role

We are seeking a skilled Nurse Care Coordinator to join our team in the Operating Room at Our Lady of the Lake Regional Medical Center. Job Summary

The Nurse Care Coordinator will work closely with our healthcare team to provide seamless coordination of patient care, ensuring efficient transitions and optimal outcomes. This role involves conducting thorough assessments, developing personalized care plans, and collaborating with patients and families to achieve their goals.

Key Responsibilities
  • Conduct comprehensive patient assessments to identify needs and develop individualized care plans
  • Collaborate with healthcare team members to ensure timely and effective care coordination
  • Communicate effectively with patients, families, and interdisciplinary teams to promote smooth transitions and optimize outcomes
  • Participate in quality improvement initiatives to enhance patient care and safety
Requirements

To be successful in this role, you will need:

  • A minimum of 2 years' experience in an operating room setting, preferably in an ambulatory surgery center
  • Additional specialty experience may be required as per facility practices (e.g., pediatric, GI)
  • Graduation from an accredited school of professional nursing
  • Active RN license and valid BCLS certification upon employment; ACLS and PALS certification within 6 months
